Fax or postal mail:

  • You can print out an application form at the 密西西比 Division of Medicaid or 密西西比's Common Web Portal website.  Then, submit your completed application to the Office of Eligibility at the Division of Medicaid.
  • Fax your application to 1-601-576-4164. (An origianl signature is not required on faxed applications.)
  • Mail your application to 550 High Street, Suite 1000, Jackson, MS 39201 or the regional office that serves the county where you live.

災害或緊急情況聲明

如果受到總統或州長聲明的災害或緊急情況影響，或者是衛生及公共服務部部長所公佈的公共衛生緊急情況的影響，您可以獲得一些額外的支持協助。

  • A 部分、B 部分和 Supplemental C 部分計劃承保福利須在指定的非合約機構提供（注意：A 部分和 B 部分承保福利必須在 Medicare 認可的機構獲得）；
